2. HOW TO ENTER: You may enter without purchase by filling in your contact information on this web page: www.buckeyecablesystem.com/footballticketgiveaway. Then hit "submit." One entry per household.
3. ALTERNATE MEANS OF ENTERING: Mail a 3x5 index card with your name, address, phone number and signature to "Thursday Night Ticket Giveaway" c/o Buckeye CableSystem Marketing, 5566 Southwyck Blvd., Toledo, Ohio 43614.
4. RANDOM DRAWING: Winner will be chosen at random from all eligible entries received by Noon on November 30, 2009. The drawing will be held on or around December 2, 2009. Odds of winning depend on the number of eligible entries received. Winners will be notified by phone and/or mail within 5 days of the drawing. Proper identification will be required upon presentation of the prize.
5. ELIGIBILITY: NO PURCHASE IS NECESSARY TO PARTICIPATE. The Promotion is open to legal U.S. residents currently residing within Buckeye CableSystem's serviceable areas within Lucas, Wood and Erie County, Ohio or Monroe County, Michigan, and who are at least 18 years old at the date of entry. One entry per household. Current and former employees, officers, and directors of Sponsor and those living in the same household are not eligible to enter or win. Subject to all applicable federal, state and local laws, regulations, and ordinances. Void where prohibited by law. Participation constitutes entrant's full and unconditional agreement to these Official Rules and to Sponsor's decisions, which are final and binding in all matters related to the Promotion. Winning a prize is contingent upon fulfilling all requirements set forth herein. Sponsor reserves the right to verify the eligibility of winners. Winner's account with Buckeye CableSystem must be in good standing.
6. Acceptance of prize constitutes permission for Buckeye CableSystem and its agencies to use winner's name and/or likeness for purposes of advertising and trade without further compensation, unless prohibited by law.
7. By accepting prize, winners agree to hold Buckeye CableSystem and Block Communications or subsidiaries and its sponsors, their respective directors, officers, employees and assigns, harmless against any and all claims and liability arising out of use of prize.
8. Sponsors are not responsible for any typographical or other error in the printing of the offer, administration of the giveaway or in the announcement of prizes.
9. Winner may be required to sign a liability release and consent waiver form to claim prizes, or prize may be forfeited and an alternate winner selected.
10. Winner may receive a 1099 tax form.
11. Participating sponsors are not eligible for late, lost, illegible or misdirected entries. All decisions of contest sponsors regarding eligibility, entries, winners, and other matters are final.
12. By participating in this promotion, entrants agree to be bound by the Official Rules and the decisions of the judges. Winners are only eligible for one grand prize from Buckeye CableSystem per 12 month period.
13. The prize packages are non-transferable.
14. No substitution, transfer or cash equivalents allowed.
